Since 1993, Yale School of Medicine has operated the Community Health Care Van (CHCV), a mobile medical clinic which, through grant funding, provides medical and harm reduction services throughout low social vulnerability index neighborhoods in New Haven, CT. In March of 2020, in partnership with New Haven Healthy Start and the March of Dimes, we launched a CHCV Mother Infant Program (CHCV MIP). This novel intervention expands the CHCV services to post-partum mothers and their newborn infants following discharge from the hospital. The CHCV MIP re-engineers the delivery of healthcare during the post-partum transition by providing “hands-on” healthcare near, but not in, the patients’ homes. Families are welcomed on to the van to discuss the CHCV MIP, complete infant physical exams, screenings for maternal hypertension, pre-eclampsia, post-partum depression, and other behavior health needs. Additionally, mothers are offered lactation assessments and a space to ask questions about newborn care and the post-partum period.
